First Visit Info at FYZICAL Bridgeville

Your first therapy visit will be about one hour. You will meet with your therapist and a treatment plan will be established so that you may meet your physical or occupational therapy goals. Each of your following appointments will be between 40 minutes and 1 hour. Your doctor and therapist will determine how many times a week you will need to attend therapy. You should arrive 15 minutes early so that the front office staff can organize your paperwork. We will also send you a link prior to your first appointment to complete your paperwork online if you prefer.

WHAT YOU NEED TO BRING

  • Insurance card/s (if using insurance)
  • Driver’s License
  • Prescription/script from your physician for therapy services, or your physician's information and we can obtain the script directly.
  • HUMANA PATIENTS PLEASE NOTE: Humana in PA requires the referring physician to fill out prescription/script details online through their site. Humana does not accept our submission of your physician's printed script. Please ensure your physician completes the prescription/script details online through Humana's designated site so we are able to submit billing on your behalf.

  • Payment is due at the time of each service

FAQ

DO WE ACCEPT MEDICARE?

Yes, we are a Medicare provider! Each year there is a flat deductible that must be met prior to Medicare covering services. Once that is met, Medicare covers 80% of physical and occupational therapy services. The remaining 20% is the patient's responsibility unless you have a supplemental insurance. As a courtesy we bill all secondary insurances. Currently there is a Medicare cap in place for outpatient therapy services, but we will work with you and Medicare to provide you with the amount of care necessary to complete your therapybut we will work with you and Medicare to provide you with the amount of care necessary to complete your therapy.

DO WE ACCEPT PRIVATE INSURANCE?

We currently are in-network with Highmark (and Blue Cross/Blue Shield plans except Together Blue), Humana, Cigna, United Healthcare (UHC), Veterans Affairs (VA) Community Care Network, Zelis, NexusACO, MPN-Empire Plan, Mid Atlantic Medical Services, Inc (MAMSI), Prime Health Services (formerly Intergroup), and also Tricare Standard (which operates non-network). Please note for Aetna we take out of network benefits. We also accept Worker's Comp, auto accident claims, and other related cases. We bill as a courtesy to our patients. It is important to contact for benefits prior to attending therapy to ensure that services will be covered. We will also call to verify your benefits for you, as programs and coverage can vary even with the same insurer.

HOW DO I KNOW WHAT MY INSURANCE WILL COVER?

As a courtesy to our patients we will call to verify your benefits for therapy services. Please remember that a verification of benefits IS NOT a guarantee of payment. We are often quoted incorrect information and can only relay what we are told by your insurance company. It is always a good idea for you to call to verify this information.

DO I NEED A PRESCRIPTION OR REFERRAL FROM MY DOCTOR?

All Medicare patients require a prescription for therapy services. Prescriptions are valid for 30 days following the date of the script. Private insurance companies may or may not require that you obtain a prescription. Some require a direct referral from your primary care physician (PCP). In all cases, it is important to call to verify this information with your insurance prior to starting treatment.

As a direct access state, patients in PA can receive therapy with us for 30 days, after which we can have a conversation with your doctor to attain a referral to continue on your healing journey.
