Bartosz Tulicki Clinical Director PT, DPT
Dr. Bart Tulicki is a Physical Therapist and the Clinical Director of Fyzical Therapy and Balance Center in Chicago. He received his Doctorate in Physical Therapy in 2014 from Franklin Pierce University in Goodyear, AZ.
Dr. Tulicki’s diverse experience, extensive knowledge, and passion for balance and vestibular therapy include special interests in treating patients with BPPV, vestibular neuritis, vestibular loss, and post concussive disorders. He uses a unique evaluation style to determine the cause of symptoms. Through the use of hands-on techniques and functional training he delivers highly effective treatment to his patients. Dr. Tulicki has lectured extensively on the treatment of patients with vestibular and balance dysfunctions in the United States and Japan.
Over the years he has developed a very encompassing and successful approach in the treatment of acute, chronic, and recurrent orthopedic and neurological disorders.
Fluent in both English and Polish, Dr. Tulicki is dedicated to providing comprehensive care to all of his patients.
Daniel J. O'Connor Physical Therapist PT
Upon graduating from the University of Illinois at the Medical Center in June of 1977 with a degree in Physical Therapy, Dan worked in various outpatient settings. The desire to provide the best possible individualized, one-on-one treatment for people with orthopedic conditions, motivated him to establish Sportho Physical Therapy, Ltd., in the spring of 1983.
Dan's unwavering commitment to providing the best possible care to those in need of physical therapy, remains his number one priority. With this goal in mind, Dan has continued with post-professional education, in order to stay abreast of new techniques, which would expedite the rehabilitation process. Additionally, Dan invested in the expansion of Sportho Physical Therapy, Ltd., in overall physical size to include the Aquatic and Work Conditioning Wings, as well as, state-of-the-art equipment.
"Though I have been practicing physical therapy since 1977, I am as passionate and devoted to those in need of physical therapy as I was when I entered the profession and treated my first patient. I remain very active in patient care and enthusiastic about the benefits of physical therapy in relation to the restoration of function in the spring of 1983. In 2014, I chose to join an excellent organization that would allow me to continue to practice Physical Therapy the way I think it was meant to be. We became a franchisee with Fyzical Therapy and Balance Centers. We can now work on builiding our business without compromise. Because we have been able to achieve, what I feel, are remarkable results through the use of Aquatics and because I want to be able to offer our form of therapy to the members of our community, we are now Fyzical Therapy and Balance Centers - Chicago."
Nancy Angel Physical Therapist PT
Nancy Angel completed her physical therapy education at the University of Illinois, Chicago in 1984. She has extensive experience in treating a wide variety of orthopedic and neurological conditions in multiple inpatient and outpatient settings. She has a special interest and strong expertise in treating amputees and training patients in the use of prosthetics. She is committed to providing individualized function-based treatments to help patients regain pain-free function and return to both the activities they need to do and those they love to do.
Outside of work, Nancy loves to travel with her husband and other family and friends. She enjoys staying active with volleyball, pickleball, golf, biking and running.
Diane Tobin Office Manager
Diane joined the staff of Fyzical (formally known as Sportho Physical Therapy) in December of 1985, prior to Managed Care, PPO's and the never-ending changes in Insurance restrictions and requirements. Having undergone extensive physical therapy, Diane knows firsthand its importance in the restoration of one's function. She continues to navigate through Insurance contracts and policies, providing our patients with a better understanding of their benefits, requirements, coverage and costs. Diane remains dedicated to securing Physical Therapy treatment for all those in need despite insurance limitations. Striving to provide a high standard of customer service from beginning to end, she is always available to answer questions and assist in any way necessary.
Will Capraro Fyzician Liaison
Will Capraro joined the Fyzical team in February of 2019. His role is to facilitate relationships between our clinic, Physicians, Otolaryngologists and other referral sources. Will graduated from the University of Colorado in 2016 with a focus in communications and marketing. “The part I love most about this job is being out in the field and meeting the many different Physical Therapists, Doctors, Nurses and Staff that help to change the lives of our patients”.
Outside of Fyzical, Will enjoys going to Chicago sports games, especially the Bears and the Blackhawks. He often goes back to Colorado to ski, hike and attend concerts at Red Rocks Amphitheater. Will enjoys spending time with his family and friends and the many different activities the City of Chicago brings.
Patty Hernandez Client Care Specialist
Patty Hernandez joined the Fyzical team in November 2017 and has thoroughly enjoyed the role of Client Care Specialist. Having worked in customer service for over two decades, she is committed to giving our patients the support and assistance they need. She takes great pride in building relationships with each and every patient that walks through our doors. From initial evaluations through the entirety of each patients unique, individualized plan of care, Patty will assist with any questions or concerns you may have.
