Pelvic Floor Physical Therapy for Menopause:  A Missing Link in Midlife Health!
Menopause is a natural transition, but that doesn’t mean you have to suffer through it. For many women, this stage brings surprising and often frustrating changes: bladder leaks, pelvic discomfort, painful sex, and core weakness. These symptoms aren’t just “part of getting older”—they’re signs your body needs support.

As a pelvic health physical therapist, we specialize in helping women navigate menopause with confidence, control, and strength. If you're experiencing any combination of vaginal dryness, urinary urgency, discomfort during intimacy, or core instability, pelvic floor physical therapy may be the missing link in your care.

Understanding the Pelvic Floor During Menopause

The pelvic floor is a group of muscles that support the bladder, uterus, and bowel. These muscles also play a critical role in sexual function, stability, posture, and continence.
During menopause, estrogen levels decline—and that decline affects more than just your mood or periods. Estrogen keeps your vaginal tissue supple, your bladder lining strong, and your pelvic floor responsive. Without it, you may notice:
- Urinary incontinence (leaking when you sneeze, laugh, run, or when rushing to bathroom)
- Pelvic organ prolapse (heaviness or bulging in the vaginal area)
- Painful sex due to vaginal dryness or muscle tightness
- Constipation or bowel changes
- Core weakness or back pain
These are incredibly common, but they are not normal—and they are certainly not untreatable.

How Physical Therapy Helps in Menopause
Pelvic floor physical therapy offers a holistic, evidence-based approach to managing genitourinary symptoms of menopause (GSM) and helping women feel more connected to their bodies.
Here’s how a pelvic health PT plan typically supports women in midlife:
1. Evaluation and Education
Every client begins with a full assessment, including an internal pelvic floor exam (with consent), posture and movement screening, bladder and bowel habits, and sexual health history. We then explain how hormonal changes affect tissue and how therapy can restore function and alleviate discomfort.
Education alone is empowering. Many women tell me, “I had no idea this was treatable without surgery or medication.”
2. Bladder and Bowel Retraining
Leaking, urgency, or constipation can stem from poor pelvic floor coordination or muscle overactivity. We work to improve muscle timing, breathing, and pressure management so you can regain control and predictability in your daily life.
3. Vaginal Tissue Support and Pain Management
If you’re experiencing pain with intercourse or using tampons, or if your GYN has diagnosed vaginal atrophy, we gently introduce tools like vaginal dilators, lubricants, and pelvic floor release techniques to help reduce pain and reintroduce comfort and function.
4. Strength and Stability
Menopause often brings changes to balance, posture, and core strength. Our treatment includes safe and progressive strengthening of the pelvic floor, hips, glutes, and deep core muscles, restoring your ability to lift, carry, and move without worry.
5. Sexual Wellness and Intimacy Support
Pain with intimacy is more common than most people realize during menopause. Physical therapy can help reduce tension, restore blood flow, and improve confidence in your body—so you can enjoy connection without fear or discomfort.

Real Women, Real Results
Clients often tell me their GYNs said, “This is just part of aging.” But once they complete therapy, they feel like themselves again—stronger, more confident, and more in control.
Whether you’re newly in perimenopause or postmenopausal for years, it’s never too late to address these issues. One of my favorite clients recently said, “I thought my body was just falling apart. Now I know I just needed the right kind of help.”
