Student Observers
DRESS CODE
For Men: Polo and/or golf shirt & comfortable dress slacks, as well as, closed toe shoes and/or neat looking tennis shoes are acceptable.
For Ladies: Blouse, polo and/or golf shirt & comfortable slacks, as well as, closed toe shoes and/or neat looking tennis shoes are acceptable.
*Please Note: No blue jeans, shorts, skirts, t-shirts, hats and/or open toe shoes will be allowed. Please note any violation of the DRESS CODE, as stated above, will result in you being asked to come back at a later date.
BEFORE your first day observing at FYZICAL (formerly HeathQuest) you will need to print off & sign the Student Observer HIPAA (Privacy Act) Release Form. You will then need to download & view the Student Observer HIPAA Training, as well as, download, print off & complete the Student Observer HIPAA Trainging Quiz.
Please remember you MUST bring with you BOTH the completed & signed Student Observer HIPAA Release Form & the completed Student Observer HIPAA Trainging Quiz, before you can legally be permitted too observe.
