Cancelations and Re-schedules made less than 24hrs in advance, we reserve the right to charge a $50 fee.
No-shows for initial evaluations and daily appointments, we reserve the right to charge an $80 fee.
Late arrival to appointments and the clinician is unable to see you due to the time, it may be considered a no-show and we reserve the right to charge an $80 fee.
This fee will be due prior to treatment at your next scheduled appointment.
We appreciate your diligence to communicate with us and attend each of your therapy sessions as we continue to work with you in your recovery.
Credit Card on File Policy

Fyzical Therapy and Balance Centers will be enforcing a Credit Card on File Policy. All patients will acknowledge and sign a document covering all requirements, notices, alternative payments, etc. regarding this Policy.
Purpose: To comply with insurance requirements and ensure timely, accurate billing, Spectra Sports Medicine & Physical Therapy LP dba Fyzical Therapy & Balance Centers Lubbock and Southwest Lubbock (Fyzical) requires all patients to maintain a valid credit or debit card on file.
