First Visit Info at FYZICAL Northwest Tucson
Your first therapy visit will be 60 minutes. You will meet with your therapist and a treatment plan will be established so that you may meet your physical therapy goals. You should arrive 10 minutes early to ensure payment and paperwork is in order. We can't wait to see you!
WHAT YOU NEED TO BRING
- Driver’s License
- Filled-out intake forms
- Bring any other helpful information if applicable (doctor's note, surgery information, etc.)
WHAT YOU WILL NEED
TO WEAR:
- Loose, comfortable clothing
- Bring or wear appropriate shoes for exercising (if applicable)
PAYMENT
- Payment is due at the time of each service
- The front office staff will advise you of your financial responsibility on your first visit, if not before
- We accept cash, checks, debit cards, Visa, Mastercard, and Discover
FAQ
DO WE ACCEPT INSURANCE OR MEDICARE?
No, we are a cash-based practice and do not bill through Medicare or other insurance providers. However, upon request, a Superbill can be made, which is a detailed document of provided services that can be submitted to an individual's insurance by the individual to determine eligibility for reimbursement.
DO I NEED A PRESCRIPTION OR REFERRAL FROM MY DOCTOR?
No, here we offer direct access which means you do NOT need a referral from your doctor. If you do receive physician orders and choose to utilize our therapy services, you may bring your referral or other information for us to review.
