First Visit Info at FYZICAL Northwest Tucson
Your first therapy visit will be 60 minutes. You will meet with your therapist and a treatment plan will be established so that you may meet your physical therapy goals. You should arrive 10 minutes early to ensure payment and paperwork is in order. We can't wait to see you!
WHAT YOU NEED TO BRING
- Driver’s License
- Filled-out intake forms (if applicable)
- Bring any other helpful information if applicable (doctor's note, surgery information, etc.)

- Loose, comfortable clothing
- Bring or wear appropriate shoes for exercising
PAYMENT
- Payment is due at the time of each service
- We accept credit cards, debit cards, cash, and checks

DO WE ACCEPT INSURANCE OR MEDICARE?
No, at FYZICAL Northwest Tucson, we are a cash-based practice and do not bill through insurance. We are not enrolled as a Medicare provider, which means Medicare will not reimburse for services received at our clinic.
We’re happy to give you a receipt for your records, but please note:
Physical therapy is a service that Medicare does cover when it is provided by a Medicare-enrolled clinic. If you prefer to seek care at a facility that is enrolled, we completely respect your choice.

DO YOU OFFER SUPERBILLS FOR NON-MEDICARE PATIENTS?
If you are not enrolled in Medicare, we can provide you with a superbill after your visits, so you can submit to your insurance company for possible out-of-network reimbursement. We clearly note that your visits have been paid in full, so any reimbursement should be sent directly to you.

At FYZICAL NW Tucson, our model is different from many traditional physical therapy clinics. In many insurance-based settings, therapists are often required to see multiple patients at the same time and rely heavily on aides or passive treatments to meet billing standards. Instead of following insurance-driven requirements, we focus on individualized, one-on-one care that puts your goals first. At FYZICAL, your entire session is spent with your physical therapist, allowing us to focus only on treatments and exercises that accelerate your recovery and long-term success.
DO I NEED A PRESCRIPTION OR REFERRAL FROM MY DOCTOR?
No, here we offer direct access which means you do NOT need a referral from your doctor. If you do receive physician orders and choose to utilize our therapy services, you may bring your referral or other information for us to review.
