Texas HB 4099: Expanding Direct Access to Physical Therapy to 30 Days

🚨 What Just Happened?

On June 20, 2025, Governor Abbott signed House Bill 4099, officially extending the period during which Texans can access physical therapy without needing a referral—from the previous 10 business days to 30 calendar days, effective September 1, 2025

🩺 Why This Law Matters

HB 4099 removes administrative burdens and accelerates patient care by allowing a longer direct-access window to start treatment—plants, sprains, post-op recovery, and chronic pain, no referral required for up to a month .

📌 Who’s Eligible?

Licensed physical therapists who:

  • Have practiced at least one year,

  • Carry professional liability insurance,

  • Hold a doctoral degree or completed ≥30 hours of differential-diagnosis training

This simplifies previous tiered rules and ensures consistent access across therapist certifications.

After 30 days, a referral will be required to continue care.
