FREQUENTLY ASKED QUESTIONS

WHAT TIME SHOULD I ARRIVE?

Please arrive at least 15 minutes prior to your scheduled appointment time. 

WHAT DO I BRING TO MY APPOINTMENT?

Upon arrival you will be greeted by a Patient Representative and asked to provide your driver’s license, insurance cards and your patient information forms (if you chose to download them and fill them out).

ARE THERE OTHER FORMS I WILL BE REQUIRED TO FILL OUT?

Depending on the condition you are being treated for, you may also be asked to fill out an injury specific form that will aid your therapist in the treatment of your condition.

HOW LONG IS MY VISIT?

Your first visit typically lasts 45 to 75 minutes depending on the severity and complexity of the condition for which you are being treated for.  Subsequent Visits: Your treatment sessions (after your first visit) will on average last 45 to 60 minutes, depending on the severity and complexity of your condition.

WHAT DO I WEAR?

It is highly encouraged that you wear comfortable loose fitting clothes that are modest in nature to allow participation in active treatment programs. For safety and hygiene, lace up, closed toe shoes are highly encouraged. At times, you may be required to change into shorts & t-shirt or a gown to allow the therapist to properly treat you.
