Insurance Information

We understand that answering insurance and cost questions is important.  We provide information here to help you get those answers.  Ultimately, only your insurance company can definitively tell you your benefits, copayment, deductible, and paperwork you may need to provide.  

We work with most insurance companies and payors that are will pay on your behalf.  We also accept major credit cards and have Care Credit, including for when you need care but have not fulfilled your deductible.

• Ask if a referral is required for evaluation (not required by law; some insurance plans require it)

Medicare patients are welcome!  Here is a public list of your available covered services.  Our services are generally physical therapy, but note that balance diagnostic coverage is also available.
