First visit info for FYZICAL - San Francisco
Your first therapy visit at FYZICAL Therapy and Balance Centers - San Francisco will be approximately one hour. You will meet with your therapist, and a treatment plan will be established so that you may meet your physical therapy goals. Each of your following appointments will be between 45 minutes and an hour. Your therapist will determine how many times a week you will need to attend therapy. Before your first appointment, you should arrive 15 minutes early so that the front office staff can organize your paperwork and make the necessary copies of your insurance cards.

Do we accept Medicare?
Yes, we are a Medicare provider. Medicare covers 80% of physical therapy services. The remaining 20% is the patient's responsibility unless you have supplemental or secondary insurance. As a courtesy, we bill most supplemental and secondary insurances. Currently, there is a Medicare cap for all outpatient therapy services.
Do we accept private insurance?
We participate with most health insurance plans. Please refer to the "Insurance" tab for details.
How do I know what my insurance will cover?
Please contact your insurance provider for eligibility and benefits; however, we can assist you with this step as necessary. Please remember that a verification of benefits is not a guarantee of payment.
Do I need a prescription or referral from my doctor?
FYZICAL - San Francisco requires a physician's referral for all HMO, PPO, Medicare, Workers' Compensation, and motor vehicle accident-related (MVA) patients. Medicare prescriptions are valid within 30 days of the date of the script. FYZICAL - San Francisco honors prescriptions from the following HMOs - Brown & Toland Medical Group, Hill Physicians Medical Group, and Alta Bates. We honor prescriptions from most PPOs. The HMO and PPO referrals are valid within 60 days of the date of the script.
