Welcome to FYZICAL

We know you have many options when choosing where to go to receive therapy services, and we thank you for choosing FYZICAL.  Our goal is to provide you with the most accurate and detailed information possible, so that we can ensure a great experience.  The relationships you will form with our clinical and administrative team will feel like a therapeutic alliance; all working towards the goal of helping you to move better and feel better.  If you have ANY questions about what is happening during your therapy sessions, simply ask.  We encourage questions throughout your rehab journey and promise to provide clear, concise answers about your treatments, condition and program.

First Visit Information

Your first therapy visit consists of an initial evaluation and will be approximately 40-60 minutes. You will meet with one of our therapists who will conduct a very thorough review your injury and past medical history in order to begin an effective and long-lasting restoration of your quality of life.  They will ask you questions to learn about your condition, your previous level of function and how your condition or injury is affecting your life.  They will also take specific measurements of the impairments that may be causing your problem or pain as well as perform other tests and measures to gather objective information.  An individualized treatment plan will then be established to ensure you meet your goals. Your customized treatment plan may include appropriate exercises, education, skilled hands-on therapies, prescribed modalities and initiation of a home exercise program designed to get you back to loving your life.  The rest of your treatment will be determined by the information gathered during the initial evaluation.

Please arrive 15 minutes early so that our front office staff can organize your paperwork and make any necessary copies of your health records and insurance cards.  To save time, you can also print the intake forms located on our forms page and bring them completed to your appointment.

What to Bring:

  • Printed registration form (or email to dewitt@fyzical.com, fax to 517.669.5675)
  • Insurance card(s)
  • Payment for any co-payments, deductibles, co-insurance or private-pay rates
  • Driver’s License/ID
  • Prescription from your physician for therapy services
  • Any previous medical records from physician(s)
  • Medication & allergy list
  • Water bottle and towel
  • Diagnostic imaging/surgical reports (MRI, CT, X-Ray, VNG, etc.)
  • Any questions or concerns you may have

What to Wear:

  • Loose, comfortable clothing or athletic wear
  • Clothing which allows us to evaluate your area of injury
  • Appropriate shoes for exercising (no flip-flops or sandals)

How Long Does Each Visit Take?

The initial visit takes a little longer than the subsequent visits due to the paperwork that you need to fill out, in addition to the completion of the assessment process. Normally, appointments are between 40 and 60 minutes depending on your specific needs.

How Long Will I need Therapy?

Your doctor and therapist will determine how many times a week you will need to attend therapy and your progress will be monitored by your physician.  You will need to attend therapy sessions until you and your therapist decide that you have reached your desired level of function or that further therapy would not be beneficial. 

Will I see the Same Therapist Every Time?

We will make every attempt to schedule you with the therapist you desire to see. However, there may be times when another therapist or therapist assistant will treat you to accommodate your schedule. All of our therapists and assistants are fully-licensed or certified and enjoy delivering high-quality care.

Payment:

  • Payment is due at the time of each service
  • The front office staff will advise you of your financial responsibility on your first visit, if not before
  • We accept cash, checks, debit cards, VISA, MASTERCARD and Care Credit.
